Therapeutic Approaches to Combatting Hypermetabolism in Severe Burn Injuries

چکیده

Every year, patients with burn injuries account for approximately 500,000 hospital emergency department visits in the United States [1]. While most burn injuries are minor, 40,000 hospitalizations related to severe burn injuries occur annually [1]. Severe burn injuries result in metabolic inflammatory alterations that are similar to those presented by other major trauma patients. However the severity, length, and magnitude of these changes are most profound and prolonged in patients with burn injuries [2-4]. In fact, insulin resistant, hyperglycemic, hypermetabolic state of burn injury patients was found to persist for up to three years [5]. The complex condition of hypermetabolism plays a critical role in the healing and long-term outcomes to severe burn injury in a patient. Burns which encompass greater than 20% total body surface area (TBSA) are characterized by pathophysiological stress, insulin resistance, hyperglycemia and catabolism due to factors such as enhanced glycolysis, proteolysis, lipolysis and wasted substrate cycling [3, 6, 7]. As a result, the clinical complications following severe burn injury are unique and complex [8] and patients require an elaborate and carefully executed treatment plan to combat the challenges they face.

Development of Metabolic Indicators of Burn Injury: Very Low Density Lipoprotein (VLDL) and Acetoacetate Are Highly Correlated to Severity of Burn Injury in Rats

Hypermetabolism is a significant sequela to severe trauma such as burns, as well as critical illnesses such as cancer. It persists in parallel to, or beyond, the original pathology for many months as an often-fatal comorbidity. Currently, diagnosis is based solely on clinical observations of increased energy expenditure, severe muscle wasting and progressive organ dysfunction. In order to ident...

The Hepatic Acute Phase Response to Thermal Injury

A thermal injury represents one of the most severe forms of trauma and occurs in over two million people in the United States of America per year (1). According to the World Health Organization (WHO), an estimated 330,000 deaths per year worldwide are related to thermal injury (2). Over 440,000 children receive medical attention for burn injuries each year in the United States (3). With approxi...

In vivo rabbit hindquarter model for assessment of regional burn hypermetabolism.

Severe burn injury evokes hypermetabolism and muscle wasting, despite nominally adequate nutrition. Although there is much information on whole organism and isolated tissue metabolism after burn injury, data examining regional burn hypermetabolism in vivo are lacking. Using surgically implanted (general anesthesia) regional vascular catheters and primed constant infusion of l-[1-(13)C]phenylala...

Background and objectives: Severe burn damage and its consequences are life threatening which can complicate patients’ health. Burn damage increases reactive oxygen species (ROS) generation which leads to severe damage to tissues and is implicated in burn shock. Some medicinal and traditional plants are considered as safe, natural and inexpensive sources for treatment of differ...

Long-Term Persistance of the Pathophysiologic Response to Severe Burn Injury

BACKGROUND Main contributors to adverse outcomes in severely burned pediatric patients are profound and complex metabolic changes in response to the initial injury. It is currently unknown how long these conditions persist beyond the acute phase post-injury. The aim of the present study was to examine the persistence of abnormalities of various clinical parameters commonly utilized to assess th...
